Release for Inclusion of Portrait and Biographical Information on Infoh.us

The Infoh.us website was created and is being maintained by a group of volunteer Members in the Oak Hammock community who call themselves The Online Group. It was designed to afford Members of Oak Hammock control over their personal information and a way to share that information in a way that would be useful to the Members of the community. No commercial entity is benefiting from Infoh.us and it has been created and is being maintained without charge to Oak Hammock or to Oak Hammock Members.

The website, Infoh.us, archives information about current Members of the Oak Hammock community, including only the information that Members choose to share within the community, for example, their name, address, telephone number, education, career, number of children as well as a recent portrait. If you agree, this information about you will be made available to other Oak Hammock Members in a password-protected area that blocks viewing by anyone who does not have authorization to access this area. If you prefer that your photo, phone number, etc., not appear, you will still be able to access the Infoh.us website.

In addition, with your permission, in an area of the website that can be viewed by everyone, you will be recognized during the week of your birthday and, if appropriate, the month of your wedding anniversary.

Please give your permission for us to share your portrait and biographical information with Oak Hammock members by printing and completing the Release. Deliver the document to cubby #3207.

After you print and sign your Release, we invite you to provide your biographical information (hometown, education, career, family, pets, etc.). The team will magically relocate this information so that Oak Hammock members can view it when they search for you in the "People" area.
